Registration Requirements

• Birth certificate (original)- birth card not accepted
• Shot records (updated) Louisiana certificate of immunizations
• Social security card
• 2 current proofs of residency (Entergy, cable, water, lease)
• Driver's license
• Withdrawal (drop) from previous school (1st-5th)
  
• Final report card from previous school (1st-5th)
• Residency affidavit *if applicable*
  
• **Child must be 5 before September 30th to register for Kindergarten.
